Application for Certificate of Registration as Log/Lumber Dealer

Requirements

Agent, Contractor or Dealer of Local Logs and Commercial Poles & Piles

New Application

    1. Duly accomplished application form
    2. Application Fee
    3. Registration Fee
    4. Oath Fee
    5. New Forestry Bond
    6. Articles of Partnership or Incorporation duly registered with the Securities and Exchange Commission (if applicant is a corporation or partnership)
    7. Certified copy of business/trade name with the Department of Trade and Industry (if applicant is a single proprietorship/entity)
    8. In case of private corporation, partnership or association; a copy of the latest Articles of Incorporation duly certified/authenticated by the Securities and Exchange Commission
    9. Mayor’s Permit
    10. Income Tax Return for the last (2) two years
    11. Picture of proposed Logpond duly attested by the concerned CENRO
    12. Business Plan
    13. Supply Contract (if feasible)

Renewal of Application

    1. Duly accomplished application form
    2. Forestry Administrative Fees
    3. Articles of Partnership or Incorporation duly registered with the Securities and Exchange Commission (if applicant is a corporation or partnership)
    4. Certified copy of business/trade name with the Department of Trade and Industry (if applicant is a single proprietorship/entity)
    5. Mayor’s Permit
    6. Latest Income Tax Return
    7. Supply Contract (if feasible)
    8. Sworn Statement (indicating that the payments of salaries/wages of employees and laborers are up to date)
    9. Financial Statement of profit and loss, certified by a CPA
    10. Summary Report showing the monthly logs purchases/input, production disposition/sales ending inventory report and other relevant information within the tenure of the permit; duly attested by the concerned CENRO

 

Agent, Contractor or Dealer of Imported Logs, Lumber, Veneer and Commercial Poles & Piles

New Application

    1. Duly accomplished application form
    2. Articles of Partnership or Incorporation duly registered with the Securities and Exchange Commission (if applicant is a corporation or partnership)
    3. Certified copy of business/trade name with the Department of Trade and Industry (if applicant is a single proprietorship/entity)
    4. Authorization issued by the Corporation, Partnership or Association thru
    5. Business Plan
    6. Authenticated copies of Import Documents covering the imported commodities
    7. Forestry Administrative Fees
    8. Picture of Logpond and Ownership Document of the area (in cases of imported logs and poles & piles)
    9. Picture of Warehouse and Ownership Document of the area (in cases of veneer, plywood and lumber)
    10. Certificate from the Philippine Wood Producers Association (PWPA), indicating that the type/kind and volume of the wood material(s) that the applicant intends to import, during the year, are within the importation rationalization program of “allowable capacity measure

Renewal of Application

    1. Duly accomplished application form
    2. Articles of Partnership or Incorporation duly registered with the Securities and Exchange Commission (if applicant is a corporation or partnership)
    3. Certified copy of business/trade name with the Department of Trade and Industry (if applicant is a single proprietorship/entity)

    5. Forestry Administrative Fees
    6. Latest Income Tax Return
    7. Pictures of lumberyard/establishment
    8. Proof of ownership of the lumberyard or consent/agreement with the owner

Renewal of Application

    1. Duly accomplished application form
    2. Lumber Supply Contract/Agreement from legitimate suppliers/subsisting lumber dealer
    3. Mayor’s Permit/Business Permit
    4. Annual Business Plan
    5. Forestry Administrative Fees
    6. Latest Income Tax Return
    7. Duly subscribed/sworn ending balance Inventory Report
    8. Summary Report showing the monthly logs purchases/input, production disposition/sales ending inventory report and other relevant information within the tenure of the permit; duly attested by the concerned CENRO

Application for Certificate of Registration as Importer/Dealer of Imported Wood Materials

 

Requirements

  1. Duly accomplished Application Form
  2. Authenticated copy of Business Registration as Single Proprietorship, Partnership, or Corporation from DTI or SEC
  3. Original copy of sale/supply contract between the agent, contractor & dealer, and foreign exporter of wood material at least one (1) year
  4. Authentication from the Philippine Embassy at the country of origin of the wood materials. (Translated in English/Affidavit informing that the signatory of the supplier in foreign language is the person certified by the Philippine Embassy in the country of origin)
  5. Two (2) pictures of establishment, showing the office with signage and the lumberyard/stock area
  6. Custom Accreditation with the Philippines National Single Window (NSW), optional
  7. Copy of the previous permit, if application is for renewal
  8. Original copy of the Certificate from the Philippine Wood Producers Association (PWPA) that the type or kind and volume of the particular wood material that the applicant intends to import within a current year are well within the importation rationalization program of the Total Allowable Capacity Measure
  9. Fees and Bonds (Oath, Application, Registration Fees, Cash Bond or Surety Bond)
