Application for Permit to Import Chainsaw

 

Requirements

  1. Duly accomplished application form containing the following details:
  2. Business Name Registration of Applicant from DTI, SEC Registration or CDA Registration
  3. If applicant is individual, Affidavit that he/she will use the chainsaw for legal purposes only (e.g. tree pruning, tree surgery, landscaping, timber stand improvement, and other allied silvicultural activities)
  4. Copy of Purchase orders, if imported
  5. Permit Fee

Application for Ordinary Minor Forest Products License

 

Requirements

  1. Duly accomplished Application Form or Letter of Application/Intent
  2. Application Fee
  3. Sketch map of the applied area
  4. If applicant is a corporation, association or partnership a true copy of its Articles of Incorporation/ Partnership and by-laws duly certified by the Securities and Exchange Commission (SEC)
  5. If applicant is a cooperative, a true copy of the Articles of Incorporation and by-laws duly certified by the Cooperative Development Authority (CDA)
  6. If applicant uses a trade name; a certificate of registration from the Department of Trade & Industry (DTI)
  7. Certified Copy of Latest Income Tax Return
  8. Proof of Capitalization
  9. Endorsement from the concerned Local Government Unit (LGU)

Application for Muyong Resource Permit

 

Requirements

  1. Filled-out Online Application Form or Letter of Application
  2. Certification from the Barangay Captain, indicating that the applicant is a resident of the area and practices the traditional Muyong conservation technique
  3. Management and Development Plan containing:
    1. Muyong Location & Size, Number of Plant/Tree species within the area
    2. Volume of the Raw Material requirement of livelihood project (for carving, handicraft, manufacturing, etc.)

Application for Integrated Forest Management Agreement

 

Requirements

Individual

  1. Duly accomplished Application Form
  2. Application Fee
  3. Proof of Financial and Technical Capability to undertake initial activities in forest plantation establishment and development, in the form of credit lines from financial institutions, collateral properties; or good past performance/track records in the development and management of tenurial instruments
  4. Appropriate clearance from the National Commission on Indigenous Peoples (NCIP)

Corporation

  1. Duly accomplished Application Form
  2. Application Fee
  3. Certified copy of Securities & Exchange Commission (SEC) Registration and/or Cooperative Development Authority (CDA)
  4. Articles of Incorporation and By-Laws, including the list of present offices and stockholders duly certified by the Board Secretary
  5. Audited Financial Statement for the last (2) preceding years
  6. Resolution of the corporate governing body (Board of Directors, Board of Trustees, etc.) designating the authorized representative of said corporation, association or partnership to apply/sign documents for and in behalf of the company
  7. Proof of Financial and Technical Capability to undertake initial activities in forest plantation establishment and development, in the form of credit lines from financial institutions, collateral properties; or good past performance/track records in the development and management of tenurial instruments
  8. Appropriate clearance from the National Commission on Indigenous Peoples (NCIP)

Application for Forest Land Use Agreement for Tourism

 

Requirements

Initial

  1. Duly accomplished Application Form
  2. Application Fee
  3. Map of the applied area; including the technical description, longitude and latitude coordinates, and tie point from the nearest landmark
  4. Pertinent documents showing proof that the applicant is a legitimate entity qualified to be a holder of a forestland Tenurial Instrument or Agreement
  5. For an individual applicant, certified copy of Birth Certificate or, certified copy of Certificate of Naturalization
  6. For an association, corporation, cooperative or partnership; certified copy of Securities & Exchange Commission (SEC) Registration Certificate, Articles of Incorporation/Partnership, and Resolution of the corporate governing body (Board of Directors, Board of Trustees, etc.) designating the authorized representative of said corporation, association or partnership to apply/sign documents for and in behalf of the company
  7. For cooperative, certified copy of Certificate of Registration with the Cooperative Development Authority (CDA)
  8. Indicative Management Plan
  9. Environmental Compliance Certificate (ECC), issued by the Environmental Management Bureau of DENR
  10. Appropriate clearance from the National Commission on Indigenous Peoples (NCIP)
  11. Clearance from Palawan Council for Sustainable Development (PCSD), if the applied area is located in the province of Palawan
  12. BIR Certification on the zonal valuation of the nearest commercial zone of the Barangay/Municipality or Province, whichever is higher
  13. Proof of Financial Capability to develop and manage the applied area
  14. Endorsement from the concerned CENRO, PENRO and RED of DENR
  15. Performance bond twice the annual rental of users fee as the case maybe, but not less than P10,000. Provided that 50% of the computed bond deposit may be posted in CASH and a balance in the form of surety with a duration of (5) five years renewable every (5) years in case of a lease or management agreement coterminous to the FLAg
