2. Certification from the following Local Government Unit Officials stating that the applicant is a resident of the province/municipality where Rattan area is located:
3. Proof of financial capability
4. Supply contract/marketing agreement of the Rattan Purchase Order
5. Sworn statement authorizing DENR to verify the submitted information

Corporation

1. Certified Photocopy of the Certificate of Registration from appropriate Government Agencies
2. Certification from the following Local Government Unit Officials stating that the applicant is a resident of the province/municipality where Rattan area is located:
3. Audited Financial Statements
4. Proof of financial capability
5. Supply contract/marketing agreement of the Rattan Purchase Order
6. Sworn statement authorizing DENR to verify the submitted information
