Corporation

1. Duly accomplished application form
2. Application Fee
3. Location and Sketch Map of the applied area
4. Certified copy of Securities & Exchange Commission (SEC) Registration, Department of Trade and Industry (DTI), or Cooperative Development Authority (CDA)
5. Articles of Incorporation and By-Laws, including the list of present offices and stockholders duly certified by the Board Secretary
6. Resolution of the corporate governing body (Board of Directors, Board of Trustees, etc.) designating the authorized representative of said corporation, association or partnership to apply/sign documents for and in behalf of the company
7. Indicative Development Plan, jointly prepared by the applicant and DENR Staff
