3. Letter of Credit (issued by the Central Bank of the Philippines)
4. Certified Photocopies of Income Tax Returns for the last (3) three years, for new applicants
5. Sworn statement authorizing DENR representative(s) to verify the submitted information

Licensed

1. Filled-out Online Application Form or Letter of Application/Intent
2. Photocopy of Purchase Order
3. Letter of Credit (issued by the Central Bank of the Philippines)
4. Certified Photocopies of Income Tax Returns for the last (3) three years, for new applicants
5. Sworn statement authorizing DENR representative(s) to verify the submitted information
6. Photocopy of Permit to Operate as a Wood Processing Plant, and corresponding Official Receipts

Corporation

1. Filled-out Online Application Form or Letter of Application/Intent
2. Photocopy of Purchase Order
3. Letter of Credit (issued by the Central Bank of the Philippines)
4. Certified Photocopies of Income Tax Returns for the last (3) three years, for new applicants
5. Sworn statement authorizing DENR representative(s) to verify the submitted information
6. Certified Photocopy of the Articles of Incorporation and By Laws of Partnership/Association and/or Corporation
7. Audited Financial Statements for the preceding (3) three years, for new applicants
8. Authorization Letter duly approved by the Board of Directors of the corporation, partnership and/or association designating their representative to sign and follow-up documents for and in behalf of the said corporation, partnership and/or association
