Requirements 

1. Duly accomplished application form
2. Documents supporting the legal possession/acquisition of wildlife
3. Payment Fee in accordance with DAO 2004-55 Section 9
4. Phytosanitary Certificate (for plants) or Veterinary Quarantine Certificate (for animals) from the concerned Department of Agriculture (DA) Office
